To join, please print this entire form out, complete and mail to the AXCS National Office at the address below. PLEASE PRINT CAREFULLY so we enter all your information correctly! Note: AXCS does not sell, barter, or give membership info to any entity. We only use this info to service your benefits.
- Make sure to attach appropriate payment (US Dollars only).
- One Year Membership typically runs November 1 - October 31. New members joining in the spring/summer have membership "rolled over" to the next Halloween.
- Three Year Membership is unquestionably the BEST VALUE!
